In August 2000, Zufall rear-ended a van while driving an H.R.S. truck; all four people in the van were killed.
An investigation into the wreck found that Zufall had falsified his logbooks on numerous occasions, but destruction of logbooks at the scene kept investigators from determining whether or not hours-of-service regs were violated in this crash, according to the release.
FMCSA, which assisted with the investigation, previously fined H.R.S. $31,800 for the falsified driver logbooks and for violations of drug and alcohol testing programs.
