A truck driver from Dubuque, IA, died this week when a load of hay bales slipped from his flatbed trailer and crushed him. The fatality has resulted in an investigation by the Occupational Safety and Health Administration.

According to a press release, the truck driver was getting ready to unload the hay outside of Newton, KS, on Wednesday, Aug. 3, when 13 bales slid off his flatbed, sheriff’s deputies said.

A Harvey County (KS) Sheriff’s Office spokeswoman confirmed that Dennis J. Augustin, 51, was then taken to a hospital and pronounced dead Wednesday morning.
