The FHWA-OMC toll free hotline mandated by TEA-21 is up and running. The hotline is intended to provide truckers with a means to report violations of motor carrier safety regulations and other instances of coercion or pressure to operate in an unsafe manner. The number, 1-888-DOT-SAFT (368-7238), is in operation 24 hours a day.
Truckers who call the hotline and wish to report violations will be asked to leave their names, addresses, and a phone number where they can be reached in addition to a brief description of the violation. If you will not be available by phone, ask to have a complaint form mailed to you, or follow the recorded instructions to retrieve a complaint form from FHWA's Internet site. The reporting form is on the Internet athttp://www.fhwa.dot.gov/omc/forms.htm. The forms are in .pdf format, which requires Adobe Acrobat Reader in order to view them. A link is provided so that you may download Acrobat Reader, or visit Adobe's web site at http://www.adobe.com/. You may mail a written complaint to: FHWA-OMC, Attn: Stan Hamilton HPS-10, 400 Seventh Street SW, Washington, DC, 20590. You may also fax your complaint to Stan Hamilton at 202-366-7298. Your written complaint should be concise, and include your name, address, phone number, the name and address of the violator, the date(s) of the violation(s), and any other information specific to the safety violation. It is not necessary to convey your personal opinions about the violator.
No complaints will be investigated without the name and address of the trucker making the complaint. However, a trucker who reports a violation will not be identified to the motor carrier or other violator he/she names in the complaint.
